(a) Baseline image shows subpleural reticular pattern and honeycombing (arrows), with ground-glass opacity in remainder of the lung. (b) Five months later, after 4 weeks of progressive breathlessness, progression of honeycombing and extensive new ground-glass opacification are shown. Biopsy specimen showed organizing pneumonia pattern superimposed on background UIP. Patient responded to aggressive immunosuppressive treatment.
