The pathologic findings after silica injection for inguinal hernia repair can be virtually indistinguishable from those of malignant fibrous histiocytoma. The CT appearance of a partially calcified inguinal soft tissue mass without bony destruction in older patients should raise the possibility of this uncommon entity. A history that includes silica injections will make proper pathologic interpretation and clinical management possible.
