Page 357, in the legend to Figure 1 (continued), the first two sentences should read as follows: (e) Anteroposterior venogram obtained with contrast medium injected through the left upper extremity during TVV shows the left subclavian vein (*). The left brachiocephalic vein "disappears" at the aortic arch (arrow).
