An 86-year-old man presented with painful swelling of the posterior aspect of his right ankle. Insidious swelling had been present for more than 10 years without causing pain; however, in the 10 days prior to presentation, the swelling had caused severe pain. Further history taking revealed no clues as to the cause of this swelling. At the time of presentation, radiographs (Fig 1) and magnetic resonance (MR) images (Fig 2) were obtained. Within 1 week of radiography and MR imaging, ultrasonography (US) (Fig 3) and US-guided biopsies were performed.
