Real-time MR-guided Wire Localization of Breast Lesions by Using an Open 1.0-T Imager: Initial Experience



Movie 1 (associated with Fig 2). Real-time MR-guided preoperative wire localization, with use of a freehand technique, after mastectomy and insertion of a prosthesis in the right breast of a 69-year-old woman with a history of breast cancer. The BTFE sequence (3.6/1.8; acquisition time, 0.5 second per section) shows a suspicious 5-mm lesion in the axillary tail of the right breast lateral to a single-lumen silicone implant and the tip of the needle approaching the target lesion. The needle trajectory had to be tangential to the implant surface to prevent puncture. This angulated freehand approach is impossible with lateral grid-compression plate devices. The tip of the guide wire is placed exactly at the target lesion, close to the chest wall. Histopathologic analysis revealed a benign lymph node.

Movie 2 (associated with Fig 3). Real-time MR-guided preoperative wire localization, with use of an angled stereotactic approach, in the right breast of a 32-year-old woman. BTFE sequence (3.6/1.8; acquisition time, 0.5 second per section) shows suspicious 18-mm lesion in the axillary tail that could be reidentified according to adjacent landmarks noted during a previous diagnostic examination. The lesion could not be reached with a horizontal approach owing to its location in the axillary tail near the chest wall. BTFE images during needle placement show the needle approaching the target lesion from the lateral side. The tip of the guide wire is placed exactly in the target lesion, close to the chest wall. Histopathologic analysis of the lesion revealed fibrocystic changes.
